County of Mendocino Motion Picture/Photography Permit

Below is a “Motion Picture/Photography Permit” application for the County of Mendocino. The processing fee required is $100.00 for each permit. A separate permit is required for each film, commercial, photo shoot, etc.

Please mail the “Motion Picture/Photography Permit” application (must be completed and signed) with a check or money order, made payable to the County of Mendocino, to the following address:

Please note that no permit will be issued without proof of insurance naming Mendocino County as an additional insured. Once the permit form has been approved and signed by this office, a fully executed copy will be mailed to the address appearing on the top of the “Motion Picture/Photography Permit” application. If you would like to receive a copy of the executed permit at an address other that the one appearing on the application, or would prefer to have a copy faxed to you, that information must be provided.

For the convenience of the film/photography crew, a copy of the approved permit will be faxed to the Mendocino County Sheriff’s Department and the California Highway Patrol. This does not, however, arrange traffic control or security. These tasks must be done directly with the appropriate organization prior to filming/photographing.

INSURANCE
Unless greater coverage is requested by the County at the time of permit processing, the Permittee shall furnish to the County evidence of auto and comprehensive general liability insurance, in form of certificate, covering the entire period of this permit with limits of not less than $1,000,000 combined single limit of insurance equivalent, naming the County of Mendocino, it officers, agents, employees, and servants as additional insured, and providing that the policy shall not be canceled without thirty (30) days prior written notice to County.

Permittee shall also provide legal evidence in the form of an insurance certificate or equivalent, that it has in effect for entire period of permit, broad form third party property damage insurance of at least $1,000,000, covering property that may be damage by action of the Permittee during permit period with the County as a named additional insured.

Production companies requesting to film on State owned or operated property must have a current certificate of insurance on file at the California Film Commission.

The certificate must be in U.S. $$ and include the following:
- General Liability coverage in the amount of $1,000,000 w/policy number, effective and expiration dates.
- Automobile Liability “Owned”, “Hired” and “Non-Owned” coverage in the amount of $500,000 w/policy number, effective and expiration dates.

The State of California must be named as an Additional Insured under both General and Automobile Liability policies, as well as Aircraft Liability, if aircraft will be used. For filming involving Aircraft, a separate aircraft insurance certificate must be on file that includes Aircraft Liability coverage in the amount of $5,000,000 w/policy number, effective and expiration dates.

Please use the following wording:
The State of California, including its agencies, departments, officers, agents, employees and servants is named as an Additional Insured with respect to liability arising out of operations related to filming on California State owned or operated property.
